Prolonged apnoea after suxamethonium in a Malaysian patient due to silent plasma cholinesterase--first case report.

چکیده
A bnormal variants of plasma cholinesterase (ChE, EC. 3.1.1.8) are a rarity in this region and to date there is no reported case of suxamethonium sensz#vity in the Malaysian population. We now report a case of a Malaysian Indian patient who received suxamethonium, developed prolonged apnoea and on investigation was found to be a homozygote for the silent gene. His family was screened for abnormal variants of plasma cholinesterase. The results are discussed.

Mivacurium-induced prolonged neuromuscular block.
We report a case of prolonged neuromuscular block after administration of mivacurium 0.2 mg kg-1 to a 16-yr-old patient where the duration of block was 2.5 h. The interesting points in this case were that the patient had homozygous atypical plasma cholinesterase deficiency (both parents had a normal phenotype) following liver transplantation. Investigations showed low plasma cholinesterase acti...

train-of-four on a peripheral nerve stimulator and sustained head lifting. General anaesthesia for ECT has three main functions: lack of awareness of the therapy; modification of the motor effects of the fit to prevent injury; fast recovery of reflexes and consciousness enabling early airway protection. Suxamethonium is the best neuromuscular blocking drug for ECT, because of its short duration...
متن کاملApnoea following suxamethonium: the genetic study of four generations of a family.
Nineteen subjects were investigated as a result of apnoea following the administration of suxamethonium occurring in one member of the family. Examination of the blood for cholinesterase activity, dibucaine number, and fluoride number was carried out and the appropriate genotype determined. The significance of the genetic variation of plasma cholinesterase in relation to suxamethonium apnoea is...
متن کاملPlasma cholinesterase deficiency in a neonate.
We report a two-day-old infant who had a period of apnoea lasting six hours following the intravenous administration of succinylcholine (Sch). The results of her plasma cholinesterase level and dibucaine number indicate a congenital absence of plasma cholinesterase (PChE) enzyme, although both parents and siblings had normal cholinesterase levels and dibucaine numbers. This is believed to be th...
متن کاملCharacterization of a Novel BCHE “Silent” Allele: Point Mutation (p.Val204Asp) Causes Loss of Activity and Prolonged Apnea with Suxamethonium
Butyrylcholinesterase deficiency is characterized by prolonged apnea after the use of muscle relaxants (suxamethonium or mivacurium) in patients who have mutations in the BCHE gene. Here, we report a case of prolonged neuromuscular block after administration of suxamethonium leading to the discovery of a novel BCHE variant (c.695T>A, p.Val204Asp). Inhibition studies, kinetic analysis and molecu...
